Entry Form Checklist

To Fill in the Entry Form:
1. Fill in all rider information.
- EC-SL/CE# is your Equine Canada membership #
- CAN Event OR PHTA# is your OHTA membership #
- If you do not have an OHTA membership and you are competing in a Horse Trial (not a Combined Test or Short Course) you will need to purchase a Day Membership. Here is the link for the form: http://www.horsetrials.on.ca/PDFs/2012%20Day%20Membership%20Form.pdf. You will need to submit a copy of this form and the $15 fee for each event (paid with your entry fee to the event)
2. Fill in Lindsay info for the Owner/Coach if not already completed on your copy
3. Fill in all the horse information. If you do not know some of the details, check the chart posted on the blog.
Now PHOTOCOPY the partially completed form, creating as many copies as you will need for the season.
4. Fill in the show name, date and level on each form.
5. To calculate the fees due for each show, check the omnibus. The omnibus pages can also be found online at http://www.horsetrials.on.ca/calendar.htm. There is a chart on the top right side of the form to help you add up any additional fees. Read the omnibus carefully, sometimes the fees include HST, drug fees, levies, etc and sometime you have to add them separately.
If you don’t know what level you will be competing at you can submit a blank cheque and we can fill it in closer to the show date once a decision is made. If you are not comfortable leaving a blank cheque you must remember to submit one to Raynham before opening day so we can send in all the entries together.
6. You also need to include a photocopy of all of your membership cards. Copy the side that has the membership #’s, not the pretty horse pictures! You need to attach one copy with each form.

- Signatures on the back page (Lindsay can sign his section once submitted)
- Photocopy of membership cards
- Cheque to event dated no later than opening day (post-dated cheques are not allowed)
- Cheque to Raynham Stables for $200 + HST = $226.00 for trailering and coaching.
